Forms in Sidebar

I am putting a form on my website and I am using the form maker plug in on my estate themed website. And have inserted the form from the widgets into my sidebar.

I want to put the form in the sidebar, but I only want it visible on 2 of my pages at the moment it is coming up on all of them.

Does anyone know what i need to do?????
