I can see in the screenshot the duplicated fields, but in the page you shared that is the one that has the form, I was unable to find that form. I see there is a button that says “Give us your rating” and when I click it, a form appears, but that one is not a Forminator form, and it looks different than your screenshot. Can you please share us a link where the form is located?
Also, can you try changing the loading method of the form. Go to your form, edit it, and in the “Behavior” section, at the bottom, you should see an option to load the form using Ajax. Either disable or enable that option and check if the form loads fine.
