form upload to FTP

Hi all, I am looking for a plugin which will present the user a form which they can fill out and also select one or multiple files/zip files which then when clicking submit will send the form and upload the files to a folder on the server. There were a couple out there in the wild which might have worked in the past but now seem to not be supported with WP version upgrades. An example of what I am looking for is “Easy FTP Upload
” – This functionality is for a website being created for the printing company. They would like their clients to upload their designs through their website so they can begin printing them. Any suggestions or help is much appreciated!
