Form that will create/edit page?

I’m looking for a simple plugin, or other way, maybe editor?
What I want to do is simple form that will have a dropdown to select category, textbox for some words and allow to insert a few photos. This form would create a wordpress page in sellected category, and would allow to edit it the same way that it was created.
I was looking through the plugins, but for “form + anything” query i got only tons of contact-form like plugins.
