form plugin with saved User data

Hi,

I hope someone can help me here.

I’m looking for a form plugin for dog breeding/registration that can do the folowing:

I add the dogs into the database, and users can select their dogs and register a litter from the data.
They can add names, and add more lines depending on how many cups there is.
They submit the data, and its added to the database.

Do any of you know a plugin that can do that, or contact me if it can be developed for a reasonable price.
