    Yeah it looks like your theme is applying its own CSS, giving the form its current appearance. To resolve you can apply some of your own styles via the plugin or your theme’s style.css file.

    I have made some progress but I would really like to edit the fields, is that possible and if so how? I am not up with php enough to make a custom template…is there a way just to edit the text on the form?

    It depends on how you would like to modify the fields.. if you want to change the appearance, that can be done with a bit of CSS. If you want to change the input labels, you will need to use a custom form, or use the Pro version.
