Also, could you please go to the Form > Behavior > Render and enable the Load Form Using AJAX option?

Thank you for sending the screenshot. Are you using Popups for Divi plugin, or something else for creating popups?
I’ve tested it on a lab site, and the forms seem to be working fine. If you have any site cache enabled, could you please check if flushing the cache helps?
Please also test it after enabling Load form using AJAX and Prevent page caching on form pages options in the Behavior section of the form: https://prnt.sc/LbVevwM2Jb-b
If that still doesn’t help, would it be possible for you to export and share the form with us for testing? You can export the form from the cog wheel menu in the forms list: https://wpmudev.com/docs/wpmu-dev-plugins/forminator/#forms-list
You can upload the export file to a cloud service like Google Drive, Dropbox, etc., or email it at wpsupport@incsub.com with the following subject: ATTN: WPMU DEV support - wp.org
