form not displaying ;(

    I created the form…. added the form
    code to the page….

    saved the page……

    form does not display ;(

    the written directions do not match
    what is in the dashboard.

    This form is for a not for profit and
    the page is supposed to go live in the
    morning (sept 16) for ticket sales for
    a fund raiser….

    PLEASE help
