Form issues

    Please could anyone help with a few issues that I have with a form.

    1, I need the radio buttons to be a required entry (happy for these to be check boxes if need be).

    2, I need the text boxes below each to be completed IF they answer yes to a question.

    3, I would like the check boxes to have placeholder text in there.
