Form Field Manager Field Placement

    I have purchased the Form Field Manager and am wanting to add custom fields to different placements on the form. For example, I need to add a Title field before First Name, a Middle Initial field between first and last, and then a bunch of additional fields after Email. It doesn’t look like FFM provides that much flexibility. Do I have to add the Title and MI field via custom fields code and the rest with the plugin?

    It would be great to be able to use the FFM to just build the entire layout of the form.

    Also, is there a the ability to make fields conditionally show (and conditionally be required) based on a checkbox being checked or not?
