Form Field Manager addon

Hi there,

I have purchased a donation theme for one of the NGO.

on above link–> click on “donate now” button on top right corner.

which displays a popup form.
It’s a “give plugin”.

I wanted to add more fields in it.
i have mentioned all the fields at below url

i have read your addon documention and understand that i can use form field manager add on to add new fields on donate now popup form.

I want to purchase only this add on.
and it will cost me 87$.
I just wanted to confirm two thing.
1. Am i able to add http://janvikas.in/donation-form/ form fields on donate now button popup form ?
2. it shows me 87$/ year so if i will purchae addon and add fields now, will it be available after a year to access. i understand that i will not able to add new fields after a year.
but can you pls. confirm that all the added fields will be available or not ?
