Form Customisation – Checkboxes

Hey guys,

Thanks for a great plug-in!

I was just wondering if there was an option to add checkboxes to the form?

Looking to have users be able to tick whether they are from Aus or NZ in the form, and also a checkbox for “I have read and agreed to the terms and conditions”
