Form broken

Submissions from this form suddenly stopped going to the email address which is specified in the setting since 9 March. It has been working OK for many years.

When trying to resend the Admin messages get the following error

Also when submitting get the following in the error logs

Now getting Emails failed to be sent for: 202

And it also keeps changing the date created for the form back to 2016.
