File Uploader-“Specified file failed upload test”

When I browse for an image and hit Upload, the alert “Specified file failed upload test” appears. The file appears to upload and insert just fine, so I’m happy, but I need to bring others in to use this.

On another note, this is probably a dumb question that everyone (but me) knows the answer to, but why is the inserted image wrapped in an anchor tag? Wouldn’t IMG be more effecient?
