File Upload Issue

I’ve got a form with two file uploads with each referenced in the “File attachments” section on the mail configuration page. When submitting the form the attachments are not included in either email. I’m also using Flamingo to save form submissions and there is not way to access the files in that either. The form tags and the mail config are shown below. Anything I’m missing or that looks incorrect? Appreciate any help.

[file* file-workerscomp limit:5mb filetypes:pdf|jpg|jpeg|png|doc|docx|gif|tif|tiff]

[file* file-photo limit:5mb filetypes:pdf|jpg|jpeg|png|doc|docx|gif|tif|tiff]

These are referenced in the "File attachments" section of the Mail config:
[file-photo] [file-workerscomp]
