I have tried to replicate you issue on our staging site but I am unable to replicate it. To me, the PayPal fee is added only once on the checkout page. Here is the screenshot for your reference: https://prnt.sc/s367q.
So, to troubleshoot the issue can you please inform us where you are facing this issue for only one product or for all the products? Also, it would be great if you can try deactivating all the other plugins except for the WooCommerce and the Payment Gateway Based Fees and Discounts and check whether the issue still exists or not.

As I have said previously, I tried to replicate the issue on our staging site but I am unable to replicate it. To me, it is working fine.
So, can you please try deactivating all the other plugins except for the WooCommerce and the Payment Gateway Based Fees and Discount plugin and then check whether it solves the issue or not.
