favicon.ico not working!

I’ve followed the instructions here http://codex.wordpress.org/Creating_a_Favicon
But the favicon does not show up. I’ve cleared cache even viewed the website from Zend2.com. Any ideas why a fav icon will not show up. The default WordPress favicon keeps showing up.
