Favicon and 3.9.1

FireFox nor Explorer shows up?

I’ve tried hard code in the Header/ Then removed it. I tried a couple different Plugins. This being the second with no luck.

It is a 16 x 16 png file that was uploaded by the plugin above asking for the file of “Frontend Settings” and then “Backend Settings”. In which I of course chose the same file for the both of them. PNG

Still getting an empty Favicon for a bookmark. Which I deleted the existing test bookmarks and have my Firefox set to- NOT to remember any history.

Thanks in advance for any clear steps you can share with me. I am not that hip with the programmer or webby lingo. So if you state: “Put this code in an area.” Please tell me where to find the area you reference. Thanks in advance appreciate any help on this one. I worked for a couple of hours before asking over here.
