Sorry, I can’t see your test site.
It seems conflict with other plugins.
Try to define logout url in FEP setting or disable some plugins.
I have defined all the urls in the FEP settings. But still get the error for the admin role all others seem to be okay, test site is

That didn’t work still get the following error in the Additional Capabilities section at the bottom for the admin role:
Fatal error: Call to a member function is_role() on a non-object in /home/fscbmw5/public_html/fscbook/wp-content/plugins/frontend-edit-profile/_form.php on line 184

have you updated to new wordpress?
is_role() is part of WP_Roles.
if you still got the error then your wordpress have broken.
