Fatal DatabaseQueryException on activation — FormBuilder

    Getting a fatal error immediately after activating GiveWP (free version), which crashes both the front end and wp-admin:

    My host’s support team attempted a full uninstall/reinstall and got the identical error both times, which rules out a one-off installation glitch.

    Could someone help me identify what table or data FormBuilderRouteBuilder::getUrl() is querying for at line 84? My hosting provider doesn’t offer direct database access, so I can’t inspect the tables myself — but it seems like a required table or option may be missing or malformed.
