Errors on Safari iphone

    We have a problem with our contact form on iphones. It is fine on android.
    Two weird things happen:-
    1. The text boxes after the date picker fields also show the date picker. They should show text boxes.
    2. One of the dropdown boxes won’t show any of the options

    We’ve used this plugin for different sites for years and its always worked well. Does anyone have any ideas what could be causing this?
