Error when adding php to home template

Hi there,
I’m trying to add my slider in php to header but for some reason I get a server error 500 when I add it? “The website encountered an error while retrieving http://thesbjournal.com/. It may be down for maintenance or configured incorrectly.”
I copied the php from cnhk slideshow and put it here:
