Most probably, the update did not go through correctly. Could you please access your website through FTP and delete form-maker folder from /wp-content/plugins directory?
You will be able to log in to your website admin afterwards.
Then download the plugin from here and install it manually from Plugins page. Note, that you will not lose any form data this way.

OK but if i do this i will loose all my forms??? i have a lot!
i did that and still the admin doesnt work!
