I have tested the form and I don’t see any obvious issues. Could you explain what isn’t working as expected?

I submitted your form with some test data, and the conditional logic works fine. The form passes validation. The error comes after that, when WordPress tries to send the email: Contact Form 7 gets a “mail_failed” response, which means the server refused to send it.
I also submitted the simple form on your contact page with the same test data and that one was sent without problems (so you should have a test email from “Test” in your inbox). This means mail works on your site in general, but something in the Mail settings of the conditional form is causing the server to reject it.
Could you compare the Mail tab of both forms? Look at the To, From and Additional headers fields. A common cause is a From address that is not on your own domain, like [your-email] or a gmail address. Some hosts started refusing those. The safe setup is:
From: [_site_title] wordpress@anitawebdesigner.com
Additional headers: Reply-To: [your-email]
If that doesn’t fix it, install a plugin like WP Mail Logging and submit the form again. It will show the exact error the server gives, and you can pass that on to your host.
