Hi @hugo76
I hope you are doing good today.
Based on your page source it seems you use an updated version of Forminator. Please note that with each update of any plugin on the site, there is a possibility of some conflicts and all things should be checked. Would you please run a conflict test in the first place? Please deactivate all plugins except Forminator and check if the problem is gone. If so, then enable all plugins one by one and find which one is having a conflict. If there is no positive result, switch to default WordPress theme like 2019 and see if it works.
Before this test, we recommend full site backup or running this test on the staging site.
If that will not give any positive results, please export your form, upload it into google drive or dropbox and share a link in your next reply, so that we could take a closer look at this.
Kind Regards,
Kris

Hello Kasia @wpmudev-support2
Thank you for your message.
the problem still persists.
Following Kris’s proposal, I am discouraged because it seems too complicated to me and it seems to me that it is a compatibility bug between the WordPress and Forminator versions,
I’m still waiting a while waiting for a Forminator update that would fix the problem
Kinds regards,
Hugo

Hi @hugo76
You can follow our roadmap at https://wpmudev.com/roadmap/#forminator
But in case the problem is related to a conflict between two plugins we may need to take a closer look and the Kris test would be necessary, however, could you please export your form https://wpmudev.com/docs/wpmu-dev-plugins/forminator/#import-export upload to Google Drive and share the link here so we can test the form without any plugin on our lab site.
Best Regards

Hi Patrick, @wpmudevsupport12
Thank you for your answer.
it seems to be an update problem, I updated an old payment form of the same type, which worked and which now does not work anymore and presents the same error message.
Here is the link of the first form on Google Drive

Thank you,

Hi @hugo76,
The issue is related to a recent bug reported with the Forminator Checkbox field when uppercase letters are used in values. A fix for this will be released with our upcoming update. However, an ETA on this cannot be provided. As a workaround, please try changing the Checkbox field values to lowercase and check if that resolves the issue.
I hope that helps.
