Error creating child theme

when I attempt to create a child theme from the Responsive theme, I get an SQL/PHP “Undefined variable: parent_theme_name” followed by block of error data.
I have a .jpg clip of the error screen but don’t know how to include it on the past. Any help would be welcomed.
