Empty admin menu item being created

Hello,

As per the subject: when the plugin is enabled a blank menu item is created under the Media top-level menu. The code looks like this:

<li><a href='upload.php?page=enable-media-replace/enable-media-replace'></a></li>

This happens on a couple of sites, one running php 5.6 the other running php 7.2. It doesn’t stop things running (that I can see), but it does look odd. I only noticed because the spacing under the Media sub-menu is deeper than you’d expect (and inconsistent with the other sub-menus).
