Email sending problems

Hello, I´ve done some form with Jazzy plugin but I have a problem with sending. Here is my settings. Problem is, that after click on the “Send button” nothing do with sending message or page. I think my email server for send forms emails haven´t any problems, but I don´t know what I must do for repair that problem.
Please, help me.
