email not working, only when in widget

I have used jazzy forms on 2 sites the first is rather complex but works and emails perfectly.

The second is a simple form (basic collection of data), and I have inserted it into the widget area and it displays fine.

The problem being the send email button does nothing.

If I insert the form into a page it works perfectly.

What am I doing wrong or what should I check?
