    Sorry about that,
    I resolved the issue. It seemed to be that the fields in the email are case sensitive against the “id” fields in the form. So if the ID was: Phonenumber, and in the email template I put phonenumber, it would not work.

    I’ve gone through and made sure all of the placeholders in the email template EXACTLY matched the id fields and we’re golden.
