Ecommerce Pharmacy website functionality

Hi

I have a client for whom we have built an eCommerce site where they can sell the health products they stock online.

They wish to improve on this and start selling a certain range of medications. The issue is, these medications require a questionnaire to be completed before being able to purchase. The results of this questionnaire would need to be sent to the store owner.

Does anyone know if there is a plugin that could handle this? My searches have not yet found anything suitable.
