duplicate titles

I understand that you probably wont respond. But I would not be messaging if I had any other idea what was going on so…
I am currently having an issue with the title tags that show up in google.
it will post the blog title the post title and then the blog title again like so
THE BORGEN PROJECT|BLAH BLAH-The Borgen Project
I just want
BLAH BLAH- The Borgen Project
I am aware that this is a common problem.
So I have gone into my theme which is choices and i have replaced all the code in the title tags with <?php wp_title(”); ?>
I have triple checked that the stuff inside of the SEO title tag for posts is as follows %%title%% %%page%% % %%sep%% %%sitename%%
I have also tried the force rewrite check box
And most importantly i uninstalled and then re-installed the plugin.
