Dear Author,

I have an issue. My theme has a broken donation form. http://greenassociatesaccountants.com/wish_charity_wp_demo/donation/ where as my other theme it is fine http://greenassociatesaccountants.com/greenlife_wp/donation/

I am using the same version of WordPress (4.7.4) and seamless donation version (4.0.20)

I tried by replacing the copy of the plugin folder which is working but no luck

Can you please help me.
