I am having the same issue.
@kelseadaniels – Are you using the “On Entire Site” targeting option?
@peregrine12 – You have a JS error on your home page only. It appears that only your home page is loading multiple (3+) copies of jQuery including one from google apis. That is the cause of it not working on that page. Likely your template for the home page is adding in extra <script src="" which should be avoided for WP generally speaking.

Thank you for looking into that and discovering the conflicting scripts. That is probably the problem, however my client has requested that I not use a pop up for this application. I do appreciate your responsiveness and will remember this plugin for future needs.
