I’m experiencing the same issue as well. Downloaded the plugin today, all that shows is a submit button when using the sample shortcode.
Hi there, that does sounds strange, although we’re not developing this version anymore, I’ll be looking into this over the next week (after the GDPR deadline hits) and pushing an update if required.

When you say:

Does this mean that this plugin is now end of life?
Is something replacing it?
I checked and didn’t see this issue, will look at again tomorrow.
RE the version, its not end of life, we’re just making a new major version (complete rewrite) 🙂

To be more specific about the problem:
I see a grey rectangular area with nothing in it
It has a horizontal scroll bar
If I scroll to the right, then the Submit button comes into view….

I’ve just released a new version with a fix for this.
It is the strangest thing, I used a JavaScript syntax highlighter (overkill) just for making some things red in the shortcodes in the admin pages.
What I didn’t realise (and not sure when it made a difference) is that around some shortcode examples it was adding <span>-s. Copying and pasting into the wp editor caused our shortcodes to break (you can see this html view)…
Anyway, that’s now been fixed.
Let me know if this problem persists (and make sure to clear your cache before trying to copy & paste again)
