Different thumbnails sizes appearing – not wanted

Love the plugin, however, I’m having a couple of issues. I have my thumbs set to 230px x 230px max width/height. However some of my galleries are showing smaller thumbnails.
The galleries are set in the themes “tabs” shortcode on this page

Meeting Minutes – 178×230 correct size
Treasurer’s Reports – 178×230 correct size
Run Related Docs – 58px x 74px
Business Documents – two docs are correct size one is smaller
