Did not receipt information filled

    Hello, i have problem when a user filling the form, they can succesfully send, but me as a admin did not receive correct information that the user fill
    i just receive this in my email.

    From: [text* name] <[email* email-1]>
Subject: [select* LIST"

messege:
[your-message]

    Anyone got solution for this issue?
