Are the uploaded files automatically deleted from the server when the corresponding form entry is deleted after three months?

When an entry is older than the retention period, Forminator’s privacy cleanup (Action Scheduler, by default about once per hour) loads that entry and deletes it. Deleting the entry also runs the file-cleanup routine.

Taking two routes, if the file is not available media library it will just remove it from server, else, it will remove the media from media library and server.

Does this also apply if a form entry is manually deleted?

That’s correct, you can also test it by creating a simple upload form, save in the media library, submit the form, remove the submission and check the media library, the expected behaviour is the file being removed.

As mentioned, they should be gone.

What you just need to ensure is action schedule and WordPress cron are working as expected; The manual test that I suggested before can help to determine if the functions are working well, if so, then it would be a matter of the scheduled action and privacy settings reach the due date.
