Date Not Appearing in Email

Hi! I just added a date field to my contact form. It appears to be functioning just fine when I fill out the form, but it will NOT exhibit the date in the email that is submitted via the form to me.

In the Mail tab, I have this under Message Body: “Date Needed: [date date-201]” I also tried “Date Needed: [date-201]”

But in both cases, the date entered will not display when I receive the email. Help please. Thank you!
