Data Collection Plugin

Hello

I am looking for a Plugin that will allow me to collect data from a number of different users. I have a list of topics(about 500) for which I need definitions. The idea is to publish these on a page and allow users select the ones they can deal with and supply the definition. The page would be password protected to eliminate the obvious.
Can anyone suggest a plugin that would do this for me?
