    I see.
    Have you filled a form?
    Looks all ok, but you won’t see past entries if you don’t use an existing dB (like form7 dB)

    Which form plugin are you using?

    yes i submitted a test form and i only see name, email, phone. no other field options. I need to see these other options like status drop down and the ability to view the lead.

    I am using Gravity Forms.

    hi,
    you have a dropdown menu just above the table with the label “Choose Fields to display:”

    you can choose your additional fields using this menu.
