Customizing comments with Radio Buttons

HI all – my first post and I searched pretty thoroughly. Neophyte in PHP and CSS trying to build out my first site.

I’m struggling with trying to customize the content form.

I’m trying to include the standard forms, but before it I”m trying to include a fieldset of 6 radio buttons with the values of a title (MR. Mrs. Miss, Ect.)

My goal is to have that fieldset of radio buttons appear prior to the default fields and then to take the value of the radio button and slap it infront of the author on the post after submission.

However, despite about 6 hours of trying to work through tutorials, I’ve really struggles with the best concept even of how to do this.
