Customize Tip/Donation box

Great plugin, thanks for your work!

Here are some simple things that could make it more easily customizable:

1. Apply a filter on the hook name that is used to insert, to use whatever I want (currently I need to un-hook and re-hook for a custom case)
2. A description field below the title, to add more clarity to the “donation box”
3. Have the form $settings as arguments on the filter ‘wc_order_tip_display_form’ to have more flexibility to hide/show the donation box conditionally
