Howdy!
Are you wanting to add the “Choose Size” and “Choose Date” option to your site, or make the page look exactly like the one you linked to?
If you want to add the “Choose Size” option to your product page, you can use the variable product option to create the size differences and even assign different pricing to each. More information here: https://docs.woocommerce.com/document/variable-product/
For the “Choose Date” option, you’d need to use an additional plugin for that, as WooCommerce itself doesn’t have that functionality. This option will allow you to choose the date on the checkout page: https://www.woocommerce.com/products/woocommerce-order-delivery/ but if you need to show the option on your product page itself, you’d want to look for something else.
