Custom form fields

    Hello,
    We need to add several additional fields in the “Schedule New Appointment” form/pagr. We can’t find a way to create custom form fields in the plugin settings. How can we do this as we must have at least 6 additional fields for clients to fill in before they can schedule an appointment?
