Custom Contact Form

I am using the custom contact form plug in version 4.5.2.1. I am creating a form that contains questions about three different topics. I would like to create a heading over each topic. For instance, “Questions about topic 1”, “Questions about topic 2” and “Questions about topic 3”. The options for styles in this plug in are not very intuitive and I am fairly new on here. If anyone knows whether this is possible or can suggest another plug in for this, I would greatly appreciate it.
