Creating form, need your help

    We have downloaded Form Maker for our company and having a hard time to figure it out. We need to set up the order form
    with quantity, multiple options, and value boxes showing. Will you be able to provide step by step guide on how to do it?

    Sure! Please go to Forms > Add New > New Field > Basic Fields > Number
    Forms > Add New > New Field > Basic Fields > Multiple Choice
    Forms > Add New > New Field > Advanced > Grades
