Create search to return categories by ZIP code

This is my first post to these forums, I very much hope I am posting in the correct place (or that someone would point me in the correct direction, if not.)

I would like to create a form where a ZIP Code is entered and a list of categories is returned (in this case, the risks that ZIP Code faces – earthquake, flood, etc.). Not the posts within the categories, but just the related category title/links.

I have the data in a spreadsheet as below (could create a csv file or whatnot):

ZIP Code Category ID(s)
XXXXX 12, 13, 16, etc.

I hope I have provided enough information. Thanks in advance for your help.
