Create folder and Upload file to FTP

    Hi,
    I’m looking for plugin where may I Upload files to FTP server.

    I need form with name, e-mail and files upload.
    I have to create folder on FTP server, where folder’s name is Name from Formular.
    Sorry for my english. Someone help?
