Create a Next Fixture Widget

Hi Guys,

I want to create a sidebar widget for my amateur football clubs website to display the clubs next fixture.

I don’t know how to create it so was wondering if anyone would be willing to have a bash.

I need a
Title – text field
Date – text field or calendar selection
Time – text field
Competition – Text field
Home Team – Text Field
Away Team – Text Field

Its a simple form layout. I just don’t know how to implement it.
